Hospice RN Admissions

As a Hospice RN Admissions, you will play a crucial role in representing our Hospice programs to patients, families, and others interested in hospice. Your responsibilities will include conducting pre-assessments, sharing information, and admitting patients.


  • Evaluate patients' suitability for hospice care by reviewing medical records and consulting with other healthcare professionals
  • Provide direct patient care and offer telephonic triage as required by the State Nurse Practice Act
  • Ensure appropriate management of pain and symptoms and assess patients' response to treatments and medications
  • Assess and educate patients, family members, and caregivers about the diagnosis and self-care plan
  • Explain the advantages of our hospice program and obtain informed consent from patients
  • Admit patients, develop personalized care plans, and complete required paperwork in accordance with our company policies
  • Gather data on physical, psychological, social, and spiritual factors that may affect the health of the patient and their family, and incorporate it into the care plan
  • Order durable medical equipment (DME), medications, and supplies as needed
  • Establish and maintain relationships with referral sources
  • Maintain clinical and professional competence through continuous learning and development
  • Assist with discharge planning and ensure continuity of care from facilities
  • Ensure a smooth transition for new patients to their assigned case manager after admission
  • Provide skilled nursing services according to the plan of care developed by the Interdisciplinary Group (IDG)
  • Follow instructions from the case manager within the scope of your practice
  • Report any changes in patient needs and status promptly
  • Perform any other duties assigned to meet the needs of our patients and organization


  • Preferred: Associate's degree
  • Required: Registered Nurse license
  • Minimum one year of experience in a similar role
  • Excellent observation, diagnostic, medication management, and clinical documentation skills
  • Must have reliable transportation and a valid driver's license
