The Behavioral Health Counselor works closely with the Social Work team and reports directly to the Social Work Manager. In alignment with the PACE (Program of All-Inclusive Care for the Elderly) model, the Behavioral Health Counselor is responsible for addressing the behavioral health needs of each participant, advocating for their well-being. This role requires overseeing a caseload of participants and maintaining accurate documentation.

Primary Responsibilities:

  • Conduct comprehensive behavioral health assessments to determine the needs, preferences, and goals of participants, actively participating in interdisciplinary team meetings to develop discipline-specific goals and interventions
  • Provide and document personalized behavioral health interventions as outlined in participants' care plans, including symptom management, counseling, crisis intervention, coordinating resources and services, facilitating care transitions, and offering individual and group counseling
  • Facilitate various forms of therapy suitable for participants, such as art and music therapy, psychosocial drama, counseling, and group discussions to enhance social roles and promote well-being
  • Deliver training on behavioral health symptoms and interventions to staff members

Requirements:

  • Master's degree in Behavioral Health (MSW) or Doctorate in Psychology (PhD) is mandatory
  • Licensed as an LCSW by the CA Board of Behavioral Sciences or licensed as a Psychologist by the CA Board of Psychologists
  • A minimum of five (5) years' experience in conducting therapy, administering behavioral health assessments, and managing care planning
  • Minimum of two (2) years' experience working with frail or elderly populations
