Utilizing your strong organizational skills and clinical knowledge, you will manage patient information, schedule appointments, and follow up on treatment plans. Responsibilities

  • Maintain up-to-date records and monitor patient progress in accordance with healthcare regulations.
  • Empower informed decision-making by educating patients and families on treatment options, medications, and care procedures.
  • Assist in scheduling appointments, referrals, and follow-up visits for patients.
  • Contribute to quality improvement initiatives aimed at enhancing patient care and operational efficiency.

Requirements

  • Hold a bachelor's degree in nursing, public health, or a related field.
  • Have at least 2 years of experience as a Registered Nurse in a home health setting.
  • Possess a current nursing license (if applicable) or a relevant certification in healthcare coordination.
  • Demonstrate proven experience in a clinical setting, preferably in care coordination or management.
  • Exhibit strong communication and interpersonal skills to effectively engage with diverse patient populations.
  • Work collaboratively in a multidisciplinary team environment.
  • Be proficient in electronic health record (EHR) systems and the Microsoft Office Suite.
  • Display strong organizational and time management skills to handle multiple responsibilities and priorities.
