Our client is actively seeking multiple Community Health Nurses to join their dedicated team in the Bronx and Queens, NY. This is a full-time, direct-hire opportunity for compassionate and skilled nursing professionals with 1–3+ years of clinical experience in home health, acute care, medical-surgical, or critical care settings.
As a Community Health Nurse, you will provide skilled nursing care and clinical assessments to members and their families in home and community settings. You will play a key role in care coordination, supporting members across various environments such as community centers, hospitals, nursing homes, and other care facilities as needed.
Key Responsibilities:

Take ownership of assigned cases, seek appropriate supervision, and practice within the scope of your education and experience.

Deliver professional nursing care in compliance with state laws, regulations, and agency policies, utilizing the full nursing process.

Perform comprehensive health assessments, considering physical, psychological, social, and environmental factors impacting patient care.

Accurately complete the Uniform Assessment System (UAS) for New York, ensuring appropriate sequencing of diagnoses in collaboration with primary care providers (PCP).

Actively participate in interdisciplinary team (IDT) care planning meetings, contributing to comprehensive, member-centered care plans.

Implement and regularly update individualized nursing care plans based on ongoing patient assessments and response to treatment.

Provide preventative, therapeutic, and rehabilitative nursing procedures and techniques.

Administer medications and treatments as prescribed, performing skilled nursing procedures independently and effectively.

Maintain ongoing communication with healthcare providers, members, and family caregivers to ensure coordinated, high-quality care.
Qualifications:
-
Degree from an NLN-accredited school of nursing required; BSN preferred.

Current New York State Registered Nurse (RN) license.

Minimum of 1 year of recent general clinical nursing experience in a certified home health agency (CHHA), acute care, medical-surgical, or critical care setting. Clinical experience must be within one (1) year of hire or transfer.

Physical and mental fitness to meet the demands of community nursing, including satisfactory health clearance per NYS regulations and agency policy.

Excellent assessment, clinical judgment, and communication skills.

Ability to work effectively in a dynamic, community-based environment.
