Hospice Nurse Case Manager (Hospice)

Location: Baker City, OR

We are seeking a compassionate and experienced Full-time Hospice Nurse Case Manager to join our dedicated healthcare team in Baker City. In this essential role, you will provide comprehensive and empathetic end-of-life care to patients and their families, helping to ensure comfort, dignity, and support throughout their hospice journey. The ideal candidate will bring a strong clinical background, exceptional communication skills, and a heartfelt commitment to delivering patient-centered care.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans tailored to each patient's unique needs

• Coordinate and manage ongoing patient visits, ensuring alignment with medical directives and patient goals

• Administer medications and manage physician orders promptly and accurately

• Provide guidance and oversight to LPNs, LVNs, and Hospice Aides in accordance with clinical protocols

• Collaborate with an interdisciplinary care team to address the full spectrum of physical, emotional, and spiritual needs

• Educate and support families and caregivers, offering resources and compassionate guidance during a difficult time

• Uphold the highest standards of care, compassion, and respect in every patient and family interaction

Qualifications

• Registered Nurse (RN) license required; Associate's degree preferred

• Minimum of one year of nursing experience; prior hospice experience is highly preferred

• Willingness to participate in on-call rotations as part of the care team

• Reliable transportation and a valid driver’s license are required
