• Assist patients in performing ambulatory and daily-living activities, providing supportive care as needed.
• Collaborate with physicians to evaluate assessments and test results, personalizing treatment plans to meet each patient's unique needs.
• Maintain accurate documentation of patient care services, complying with federal, state, and local regulations.
• Conduct follow-up visits for orthopedic home health patients, verifying continued eligibility for services.
• Offer necessary maintenance therapy to facilitate ongoing patient improvement.
• Travel to patient locations as necessary for job requirements.

Qualifications:

• Successful completion of an accredited program for Physical Therapist Assistants.
• Active licensure as a Physical Therapist Assistant in the state.
• At least one year of experience working as a Physical Therapist Assistant in home health care.
• Exceptional interpersonal skills with a caring and understanding approach.
• Valid driver's license and a clean driving record.
