We’re seeking a Home Health Care Manager (RN) to lead a mission-driven team providing compassionate care to elderly participants in the comfort of their homes, as well as those residing in nursing homes and care facilities. This is a critical leadership opportunity to improve care outcomes while mentoring a high-performing interdisciplinary care team.

💼 What You’ll Do:

• Manage daily operations of the home care department, ensuring care is compliant, efficient, and person-centered

• Oversee the full life cycle of team management: recruiting, onboarding, training, and ongoing development

• Monitor the delivery and documentation of care in line with participant care plans

• Coordinate and assess care delivery across multiple settings including nursing homes and board-and-care homes

• Collaborate with interdisciplinary teams (IDT) to maintain clinical excellence and service quality

🎓 Preferred Qualifications:

• Bachelor’s Degree in Nursing (or equivalent experience)

• Proven ability to navigate complex clinical cases in a home-based or long-term care environment

• Excellent communication and problem-solving skills

• Deep understanding of regulatory standards and care planning for the elderly
