Clinical Manager (Home Health)
The Home Health Clinical Manager plays a critical leadership role in overseeing the delivery of exceptional patient care within a home health setting. This position ensures clinical excellence, regulatory compliance, and operational efficiency while serving as a mentor to staff and a key liaison between patients, caregivers, and administrative teams.
Key Responsibilities
Staff Leadership and Development

Lead and support a multidisciplinary team including nurses, therapists, and home health aides.

Manage recruitment, onboarding, scheduling, and performance evaluations to maintain a high-performing care team.

Foster a positive work culture centered on collaboration, accountability, and continuous improvement.
Patient Care Coordination

Oversee the development and execution of individualized care plans in collaboration with physicians and healthcare providers.

Ensure care is timely, effective, and responsive to patient needs, with a focus on maintaining independence and well-being at home.
Regulatory Compliance and Standards

Ensure full compliance with federal, state, and local regulations, including Medicare and Medicaid guidelines.

Maintain adherence to internal policies and accreditation requirements (e.g., Joint Commission).
Quality Assurance and Improvement

Track clinical outcomes, perform audits, and identify opportunities for quality enhancement.

Implement evidence-based practices and lead quality improvement initiatives.
Patient Advocacy and Satisfaction

Serve as a primary point of contact for patient and family concerns, ensuring issues are addressed quickly and compassionately.

Promote a patient-centered care environment that respects dignity, choice, and comfort.
Clinical Documentation and Review

Oversee and review clinical documentation to ensure accuracy, timeliness, and regulatory compliance.

Support staff in maintaining thorough and up-to-date patient records using electronic health systems.
Budget and Resource Management

Assist in managing departmental budgets and resource allocation to balance cost-efficiency with clinical excellence.

Monitor staffing levels, equipment usage, and supply needs.
Training and Continuing Education

Deliver or coordinate ongoing education on clinical standards, policy updates, and home health best practices.

Ensure staff remain current on certifications and regulatory changes.
Collaboration and Communication

Partner with case managers, physicians, administrative staff, and external agencies to streamline care transitions and enhance outcomes.

Participate in strategic planning and cross-functional initiatives.
Qualifications
Education
-
Bachelor of Science in Nursing (BSN) required

Master’s degree in Nursing or Healthcare Administration preferred
Licensure
-
Active, unrestricted RN license in the state of practice
Experience
-
Minimum 2–3 years of clinical experience in home health or a similar healthcare setting

Prior experience in a supervisory or management role

Excellent communication and interpersonal skills

In-depth knowledge of home health regulations, reimbursement processes, and industry standards
