Clinical Manager (Home Health)

The Home Health Clinical Manager plays a critical leadership role in overseeing the delivery of exceptional patient care within a home health setting. This position ensures clinical excellence, regulatory compliance, and operational efficiency while serving as a mentor to staff and a key liaison between patients, caregivers, and administrative teams.


Key Responsibilities

Staff Leadership and Development

  • Lead and support a multidisciplinary team including nurses, therapists, and home health aides.

  • Manage recruitment, onboarding, scheduling, and performance evaluations to maintain a high-performing care team.

  • Foster a positive work culture centered on collaboration, accountability, and continuous improvement.

Patient Care Coordination

  • Oversee the development and execution of individualized care plans in collaboration with physicians and healthcare providers.

  • Ensure care is timely, effective, and responsive to patient needs, with a focus on maintaining independence and well-being at home.

Regulatory Compliance and Standards

  • Ensure full compliance with federal, state, and local regulations, including Medicare and Medicaid guidelines.

  • Maintain adherence to internal policies and accreditation requirements (e.g., Joint Commission).

Quality Assurance and Improvement

  • Track clinical outcomes, perform audits, and identify opportunities for quality enhancement.

  • Implement evidence-based practices and lead quality improvement initiatives.

Patient Advocacy and Satisfaction

  • Serve as a primary point of contact for patient and family concerns, ensuring issues are addressed quickly and compassionately.

  • Promote a patient-centered care environment that respects dignity, choice, and comfort.

Clinical Documentation and Review

  • Oversee and review clinical documentation to ensure accuracy, timeliness, and regulatory compliance.

  • Support staff in maintaining thorough and up-to-date patient records using electronic health systems.

Budget and Resource Management

  • Assist in managing departmental budgets and resource allocation to balance cost-efficiency with clinical excellence.

  • Monitor staffing levels, equipment usage, and supply needs.

Training and Continuing Education

  • Deliver or coordinate ongoing education on clinical standards, policy updates, and home health best practices.

  • Ensure staff remain current on certifications and regulatory changes.

Collaboration and Communication

  • Partner with case managers, physicians, administrative staff, and external agencies to streamline care transitions and enhance outcomes.

  • Participate in strategic planning and cross-functional initiatives.


Qualifications

Education

  • Bachelor of Science in Nursing (BSN) required

  • Master’s degree in Nursing or Healthcare Administration preferred

Licensure

  • Active, unrestricted RN license in the state of practice

Experience

  • Minimum 2–3 years of clinical experience in home health or a similar healthcare setting

  • Prior experience in a supervisory or management role

Skills and Competencies

  • Strong leadership and team-building abilities

  • Excellent communication and interpersonal skills

  • In-depth knowledge of home health regulations, reimbursement processes, and industry standards

  • Proficiency in EHR systems and clinical software tools

  • Sound judgment, problem-solving, and decision-making capabilities

Certifications

  • Basic Life Support (BLS) certification required

  • Home Health Care Certification preferred
