Description:

As a Physical Therapy Assistant for our home health team, you will assist patients in carrying out their treatment plans and engaging in therapeutic activities. This will involve administering manual physical exercises, guiding and supporting patients in non-manual exercises, ambulatory functional activities, and daily living tasks. Additionally, you will provide assistive and supportive quality care. In consultation with physicians, you will assess and interpret evaluations and test results and develop appropriate physical therapy treatment plans. To comply with legal and professional requirements, you will meticulously document patient care services in both patient and department records, staying informed about existing and upcoming legislation and advising management on necessary actions. Your responsibilities will include:

• Conducting follow-up visits for orthopedic patients receiving home health services
• Maintaining eligibility requirements for home health services
• Providing maintenance therapy
• Driving, as it is a mandatory requirement

• Completion of an accredited program for Physical Therapy Assistants

• Must hold a valid state licensure

• At least 1 year of experience as a Physical Therapy Assistant in a home health setting

• Excellent interpersonal skills with a strong sense of compassion

• A valid driver's license with a clean driving record
