MUST HAVES:

• Active California Physical Therapist License (or eligibility)

• Minimum 1 Year of Experience in an Acute Care Setting

• Home Health Experience Required

• Valid Driver’s License

We’re looking for a skilled and compassionate Physical Therapist to join our team in Petaluma, CA. In this full-time role, you'll deliver hands-on, personalized care that restores function, promotes independence, and improves quality of life. You’ll work closely with patients, families, and a collaborative healthcare team in both acute and home health settings.

What You’ll Do:

• Conduct thorough assessments to evaluate physical conditions

• Create and implement personalized treatment plans based on clinical findings

• Administer exercises, manual therapy, and other evidence-based interventions

• Educate patients and families on at-home care, recovery strategies, and injury prevention

• Collaborate with physicians, nurses, and other team members for coordinated care

• Track patient progress and adjust plans as needed

• Ensure accurate and compliant documentation
