Position Overview:

We are looking for a skilled and dedicated Physical Therapist (PT) to join our team in Petaluma. Your primary responsibility will be to provide exceptional therapeutic care to patients, focusing on improving their mobility, strength, and overall physical function. Main Duties:

• Conduct comprehensive assessments to diagnose physical conditions and develop personalized treatment plans.
• Implement therapeutic interventions, such as exercises, manual therapy, and modalities, that cater to individual patient needs.
• Educate patients and their families on treatment plans, home exercise techniques, and injury prevention measures.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Maintain accurate and up-to-date patient records, adhering to regulatory standards.
• Adjust treatment plans based on patient progress and feedback.

Qualifications:

• Current licensure or eligibility for licensure as a Physical Therapist in California.
• Minimum of one year experience in an acute care setting.
• Prior experience in home health care.
• Solid understanding of physical therapy principles, procedures, and standards.
• Excellent interpersonal and communication skills.
• Ability to thrive in a team-oriented environment.
• Preferably, experience in a clinical setting.
• Clean driver's license
