🩺 What You’ll Do:

• Provide high-quality direct patient care across diverse age groups and health conditions

• Conduct thorough patient assessments and document medical histories with precision

• Develop and implement individualized care plans tailored to patient needs

• Administer medications and treatments under physician orders

• Monitor and record vital signs and patient progress, identifying any changes in condition

• Collaborate with a multidisciplinary team to ensure coordinated, holistic care

• Educate patients and families on disease prevention, treatments, and home care management

• Maintain detailed and timely documentation in compliance with healthcare standards

✅ What You Bring:

• Active California RN license

• 1+ year of recent acute care experience (required)

• For Per Diem: Must have 6+ months of home health RN experience

• For Full-Time: No home health experience required

• Graduate of an accredited School of Nursing (BSN/PHN preferred)

• CPR certification

• Valid driver’s license and insured, reliable vehicle

• Strong clinical judgment, communication, and interpersonal skills
