In-Home Nursing Specialist

Location: Smithtown, NY

Responsibilities:

  • Perform comprehensive assessments of patients requiring in-home nursing care
  • Develop customized Care Plans to address individual patient needs
  • Regularly reassess patients and make necessary updates to Care Plans
  • Supervise caregiver staff assigned to each patient for optimal care delivery
  • Educate patients and their families on safety measures, best care practices, and strategies to prevent hospital readmissions
  • Develop discharge plans when patient goals have been achieved

Requirements:

  • Must possess a valid and current RN license
  • Preferred experience in skilled home health and familiarity with OASIS
  • Acute care experience is highly desired
  • Willingness and ability to quickly learn and adapt to new EMR software
