Position: Home Health RN

Location: Islip, NY

Responsibilities:

  • Evaluate patients to determine their home healthcare needs
  • Create individualized care plans to cater to each patient's specific requirements
  • Periodically reassess and modify care plans as needed
  • Supervise caregivers assigned to patients
  • Educate patients and their families on safety measures, best care practices, and strategies to prevent hospital re-admissions
  • Develop discharge plans for patients who have achieved their goals

Requirements:

  • Must possess a valid and current RN license
  • Preferably have experience in skilled home health and OASIS procedures
  • Acute care experience is highly desired
  • Must be willing to learn and adapt to new EMR software
