We are currently looking for a compassionate and dedicated Registered Nurse (RN) to join our team in Burnsville. As an RN in our home care department, you will have a crucial role in delivering high-quality and patient-centered care to individuals within the comfort of their homes, promoting their independence and overall well-being.

Primary Responsibilities:

• Conduct comprehensive assessments to evaluate the health status of patients and create personalized care plans.
• Administer prescribed medications, treatments, and procedures as directed by the physician.
• Monitor the progress of patients and adjust care plans as needed to ensure optimal health outcomes.
• Educate patients and their families about managing illnesses or injuries and encourage healthy lifestyle choices.
• Collaborate with other healthcare professionals to coordinate patient care and ensure uninterrupted services.
• Maintain accurate and timely documentation of patient care activities and progress notes.
• Ensure compliance with all applicable state and federal regulations and adhere to established standards of care.

Requirements:

• Must hold a valid and unrestricted RN license in the state of Minnesota.
• Preferably possess a Bachelor's degree in Nursing (BSN).
• At least one year of previous nursing experience is required.
• Home health experience is mandatory.
• Strong clinical assessment skills and critical thinking abilities.
• Excellent communication and interpersonal skills.
• Ability to work independently, managing time effectively.
• Reliable transportation and a valid driver's license are necessary.
