Job Description

We are seeking a skilled and motivated Pharmacist to join our dynamic team. Our nationwide specialty pharmacy is founded and led by pharmacists who are dedicated to providing specialized care for patients with chronic and rare illnesses. This position is located near Nash, TX and offers opportunities for growth and development.


The Pharmacist will be responsible for performing a variety of distributive and clinical pharmacy activities in order to provide safe and effective home infusion therapies to our patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Comply with the policies and procedures of the organization.
  • Create and enforce policies and procedures that ensure safe handling of infusion therapies at various stages, including transportation, in the pharmacy, at the suite, and in the patient's home.
  • Collaborate with nursing, customer service, and distribution teams to coordinate and facilitate new referrals and admissions.
  • Review and verify new patient orders, documenting them accurately on the designated order form.
  • Assist physicians and other healthcare professionals in developing and implementing safe and effective drug therapy plans for patients receiving care at home.
  • Conduct thorough assessments of patients, drugs, and diseases to identify any existing or potential drug-related issues or needs.
  • Work with physicians and nursing staff to establish therapeutic objectives for drug therapy and set parameters for monitoring its safety and efficacy.
  • Monitor and evaluate the outcomes of drug therapy by collecting and interpreting laboratory data, clinical data, and other relevant patient information.
  • Provide feedback on therapeutic drug monitoring activities to the appropriate parties, including physicians, nursing staff, and other relevant individuals.
  • Program infusion control devices when necessary.
  • Offer consultation and answer inquiries from patients, families, physicians, and other healthcare professionals regarding drug use and related matters.
  • Maintain comprehensive and accurate documentation of all clinical activities and communication with patients, physicians, and other healthcare professionals.
  • Prepare progress reports for referring physicians as required.
  • Thoroughly check and process medications, supplies, and equipment prior to dispensing them from the pharmacy.
  • Stay up-to-date with clinical skills and pharmacotherapy knowledge by reviewing literature and attending relevant in-services, seminars, and conferences.
  • Participate in the on-call rotation.
  • Provide marketing support when needed.
  • Contribute to the orientation and training programs for new and current staff members.
  • Participate in departmental quality assurance activities.
  • Attend scheduled nursing department meetings.
  • Maintain pharmacy records in compliance with applicable laws and regulations.
  • Handle customer complaints according to company policies and promptly notify management.
  • Educate referral sources about our services and how they meet customer needs and benefits.
  • Complete and submit all required reports within the specified timeframes.
  • Maintain organized and updated files and records as instructed by the company.
  • Collaborate with Senior Management to enhance efficiency and product knowledge in alignment with company goals.
  • Assist in special assignments, such as market tests and surveys, as directed by management.
  • Deliver high-quality services to home-care patients and referral sources.
  • Attend conventions, trade shows, and in-services as deemed appropriate by management.
  • Represent the company in a professional manner at all times.
  • Foster effective working relationships and cooperate with colleagues and company personnel.
  • Adhere to the company's compliance requirements as outlined in the policy and procedure manual and other related policies.
  • Perform any other assigned duties and responsibilities.

QUALIFICATION REQUIREMENTS

EDUCATION AND/OR EXPERIENCE

  • Hold a Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy preferred) obtained from an accredited college of pharmacy.
  • Currently hold a valid registered pharmacist license in the state of employment.
  • Have a minimum of 2 years of experience in specialty pharmacy or ICU/critical care, with a strong preference for prior experience in plasma-derived therapy.
  • Capable of assuming responsibility for evaluating and monitoring drug therapy in collaboration with physicians, nurses, and other healthcare professionals to address the total needs and problems of home care patients.
