Home Training Nurse – Dialysis – Full-time

Home Training Nurse (Peritoneal Dialysis or Home Hemodialysis)

The Home Training Nurse plays a vital role in educating and supporting patients and their care partners in safely performing dialysis treatments at home. This role combines patient instruction, clinical monitoring, and coordination with an interdisciplinary care team to ensure successful home dialysis outcomes. The position is based primarily in dialysis clinics or home dialysis centers, with occasional travel to patients’ homes for training as needed.

Key Responsibilities:

Patient Education and Training:

  • Instruct patients and their care partners on performing Peritoneal Dialysis (PD) or Home Hemodialysis (HHD), including setup, operation, and maintenance of equipment

  • Teach infection control practices, catheter care, fluid balance, and how to recognize and manage complications

  • Adapt teaching methods to patient learning styles, ensuring competence and confidence in performing dialysis independently

  • Educate on dietary, medication, and lifestyle considerations that support treatment success

Clinical Care and Monitoring:

  • Assess patient health status before, during, and after training; monitor vital signs, labs, and treatment responses

  • Administer dialysis treatments in-center during training or at home if necessary

  • Recognize and address adverse reactions, and communicate with the nephrology team when appropriate

  • Maintain detailed documentation of training progress and clinical status

Equipment and Home Setup:

  • Work with technical teams to ensure patients' homes are properly prepared for dialysis (electrical, plumbing, drainage)

  • Confirm equipment is set up correctly and functioning properly

  • Teach patients how to troubleshoot common technical issues

Ongoing Support and Follow-Up:

  • Conduct regular follow-ups via phone or home visits to monitor compliance and address questions or concerns

  • Offer emotional support to help patients adapt to home-based care

  • Collaborate with nephrologists, dietitians, social workers, and others to tailor care plans as needed

Administrative and Compliance Duties:

  • Keep accurate records of training sessions, patient evaluations, and clinical interactions

  • Ensure adherence to infection control, safety protocols, and Medicare/clinic regulations

  • Participate in quality improvement initiatives and contribute to audit readiness

Qualifications:

  • Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program

  • Licensure: Active RN license in the state of practice; multi-state license may be needed for some roles

  • Experience:

    • Minimum 12 months of RN experience

    • At least 3 months of dialysis experience required; PD experience required

    • In-center and HHD experience preferred

    • In Texas and some states, 6 months of modality-specific experience may be required

  • Certifications:

    • Basic Life Support (BLS) required

    • Advanced Cardiac Life Support (ACLS) preferred

    • CDN or CNN certification through NNCC is a plus
