Job Summary:

We are seeking a compassionate and experienced Medical Social Worker to join our team in providing psychosocial assessments and counseling services to hospice patients and their families. Working under the direction of the attending physician, you will collaborate with the Interdisciplinary Group/Team (IDG/T) to coordinate care and ensure that patients receive holistic, tailored support. This role also includes participation in an on-call rotation as needed.

Key Responsibilities:

• Conduct thorough psychosocial assessments to evaluate the emotional, social, and environmental factors influencing patients and families.
• Offer emotional support and counseling to individuals and families facing end-of-life challenges.
• Collaborate with the IDG/T to address all aspects of patient care.
• Assist families in understanding and managing grief and loss.
• Help families access community resources and services.
• Maintain accurate documentation of social work services in compliance with hospice policies and regulatory requirements.
• Participate in the on-call rotation as necessary.

• Education:
  • Master’s degree in Social Work (MSW) from an accredited school of social work, OR
  • Bachelor’s degree in Social Work from an accredited institution, OR
  • Bachelor’s degree in Psychology, Sociology, or a related field, with MSW supervision
• Licensure:
  • Must possess current state licensure as a social worker, LCSW, LMSW, or equivalent. Preferably licensed in MO and KS.
  • Must have a valid driver’s license and reliable transportation.
• Experience:
  • At least two (2) years of relevant experience in a healthcare setting, preferable in hospice or palliative care.
  • Strong working knowledge of community resources and support services.
• Skills:
  • Excellent interpersonal and communication skills.
  • Proficient in computer usage for documentation and communication purposes.
  • Exudes compassion, empathy, and professionalism in all interactions.
