We are seeking a compassionate and dedicated individual to join our team in La Grande as a Master's Level Hospice Social Worker. In this role, you will play a vital part in providing support and assistance to patients and their families who are navigating the challenges of terminal illness.

The ideal candidate will:

• Provide counseling and emotional support to help patients and families cope with the emotional challenges and grief that come with end-of-life care.
• Collaborate with a diverse team of healthcare professionals, including physicians and nurses, to create and execute personalized care plans.
• Act as a mediator and facilitate communication between family members and healthcare providers to address concerns and resolve conflicts.
• Coordinate various treatments, services, and resources to improve the overall quality of life for patients.
• Advocate for patients by connecting them with appropriate resources and support systems.
• Offer assistance to families in managing the stresses and difficulties associated with terminal illness, including financial struggles.

Requirements:

• Must hold a Master's Degree in Social Work.
• Previous experience in a social work capacity is essential.
• Reliable transportation is a must.
