Hospice Nurse Case Manager

Hospice Nurse Responsibilities:

• Supervising the care of patients approaching the end of life.
• This includes, but is not limited to, scheduling patient visits based on their needs, collaborating with physicians, managing orders, and overseeing medication.
• Providing guidance to LPNs/LVNs/Hospice Aides as per protocol.
• Collaborating with a team of healthcare professionals to ensure that these individuals receive comprehensive palliative care encompassing physical, emotional, and spiritual support.

Qualifications:

• Preferred: Associates degree
• Required: Registered Nurse license
• Minimum one year of experience
• Prior experience in a hospice setting
• Willingness to be available on-call
• Must possess a reliable means of transportation and hold a valid driver's license
