Responsibilities of a Hospice Nurse Care Coordinator:
- Overseeing and coordinating the care of patients approaching the end of life.
- This includes, but is not limited to, scheduling patient visits as per their needs, collaborating with physicians, managing orders, and overseeing medication management.
- Supervising LPN's / LVN's / Hospice Aides according to established protocols.
- Working collaboratively with a team of medical professionals to ensure that patients receive top-quality palliative care, encompassing physical, emotional, and spiritual support.
Qualifications:
- Preferred: Associates degree in nursing or related field.
- Required: Registered Nurse license.
- Minimum of one year of healthcare experience.
- Previous experience in hospice care.
- Willingness to be on-call when necessary.
- Must have reliable transportation and a valid driver's license.
