Hospice RN – Clinical Liaison
Location: Saint Paul, MN
We are seeking a caring care coordinator to promote our facility's services to patients, their families, and caregivers. The care coordinator will ensure seamless care for patients transitioning to our facility and provide informed recommendations based on their knowledge of our services and industry updates.
We are looking for compassionate clinical liaisons who will be involved in hands-on care and social work. The role of the clinical liaison will involve rotating between units to provide hospice care in various locations.
Responsibilities of a Clinical Liaison:
- Developing and implementing marketing strategies to achieve sales targets.
- Building strong relationships with sources to generate repeat referrals.
- Participating in marketing education provided by the facility to ensure comprehensive understanding of available resources.
- Conducting facility tours and delivering presentations to potential referral sources and patient families.
- Attending seminars and conferences to promote the facility.
- Collaborating with facility staff on referrals, evaluations, and marketing initiatives.
- Evaluating physicians' orders and patients' clinical needs, and collecting relevant clinical information.
- Performing assessments to determine the suitability of potential admissions.
- Communicating activities and concerns to the supervisor.
Requirements for a Clinical Liaison:

- Obtained a bachelor's degree in nursing
- Possess at least one (1) year of RN experience within the local market, with a strong understanding of governmental home health regulations, Medicare requirements, and care plan development techniques
- Background in clinical nursing (preferably in a fast-paced ICU or ER setting)
- Previous experience in medical sales is preferred (with a sales-oriented mindset)
- Hold an active clinical license
- Possess a valid driver's license and up-to-date insurance
