As a member of our team, you will play a crucial role in providing mental health therapy and support to our participants. Working closely with the inter-disciplinary team (IDT), you will assess participants' psychosocial needs, develop care plans, provide counseling and case management services, and act as a liaison between participants, their families, support network, and the care team.

Responsibilities:

• Conduct comprehensive assessments to determine participants' psychosocial needs and goals
• Participate in IDT meetings to develop personalized care plans
• Deliver and document social work interventions according to participants' care plans
• Coordinate smooth transitions between different care settings
• Facilitate care conference meetings to ensure effective coordination among care team members
• Provide discharge planning when participants leave the program

Requirements:

• Master's Degree in Social Work (MSW) required
• At least one year of experience working with frail or elderly populations
• Prior experience in coordinating and facilitating care conference meetings
• Experience with behavioral health and substance abuse is preferred
