LCSW - Licensed Clinical Social Worker
Location: San Jose, CA

Make a difference in the lives of seniors by providing compassionate, coordinated care! We are seeking a dedicated LCSW - Licensed Clinical Social Worker to join our team in San Jose, CA. Reporting to the Social Work Manager, you will maintain a caseload of participants and work closely with the Interdisciplinary Team (IDT) to deliver comprehensive social work services that support each participant's well-being and independence.

Key Responsibilities:

• Conduct thorough psychosocial assessments to determine participants’ needs, preferences, and goals.

• Actively participate in IDT meetings to develop and update individualized care plans.

• Deliver social work interventions as outlined in care plans, including resource coordination, service referrals, counseling (individual and group), and case management.

• Facilitate smooth care transitions between healthcare settings (hospitals, skilled nursing facilities, home) in collaboration with the care team and primary care physician.

• Organize and lead care conference meetings to ensure effective communication between participants, their families, caregivers, and healthcare providers.

• Provide discharge planning support when participants disenroll from the program.

• Serve as a liaison among participants, families, support networks, and the care team to promote optimal care outcomes.

Qualifications:

• Master’s Degree in Social Work (MSW) required.

• At least one (1) year of experience working with a frail or elderly population.

• Experience coordinating and facilitating care conferences.

• Experience assisting individuals with behavioral health and substance abuse issues is preferred.
