LICENSED THERAPIST

Salary: $95,000.00 - $130,000.00 per year
Location: Palm Springs, CA
Signing Bonus: $15,000
Full-time position with Full Benefits + Paid Relocation
Hybrid Remote (2 days remote 3 days in office)

As a Licensed Therapist, you will play a vital role in coordinating care and supporting patients on their journey towards optimal health. Your responsibilities include assessing patients for transition needs, conducting psycho-social assessments, promoting timely discharges, and preventing readmissions. You will adhere to national standards for case management and provide education to healthcare professionals and patients.

Key Responsibilities:

• Manage transitions to ensure appropriate length of stay, prevent readmissions, and enhance patient satisfaction
• Coordinate care, ensuring it is sequenced and provided at the appropriate level
• Comply with state and federal regulations, TJC accreditation standards, and Tenet policy
• Educate physicians, patients, families, and caregivers
• Mentor new staff members and serve as a resource for the entire team
• Participate in department Quality Improvement initiatives and committee projects

You will also be involved in complex psycho-social transition planning, assistance with adoptions and abuse/neglect cases, care coordination, implementation of transition plans, leading patient care conferences, making appropriate referrals, communicating with patients and families, collaborating with healthcare professionals, providing patient education, maintaining accurate documentation, and other assigned duties.

QUALIFICATIONS:

• Required qualifications include a valid CA LCSW license
• Preferred qualifications include Accredited Case Manager (ACM) certification and a minimum of two years of acute hospital experience
• Must have demonstrated organizational skills, excellent communication skills, ability to lead and coordinate activities, critical thinking and problem-solving abilities, and computer literacy
