Certified Hospice Social Worker - Carmel, IN

Job Summary:
Our primary function is to deliver coordinated care to patients of all age groups in their homes. As a Certified Hospice Social Worker, your responsibilities will include conducting psycho-social assessments, analysis, counseling, and referrals to meet the unique needs of each patient and their family.

What You Will Do:

• Evaluate hospice patients to identify their psychosocial, grief, financial, environmental, and community needs. This will involve reviewing POC documentation, clinical records, team meeting minutes, and making appropriate community resource referrals.
• Provide individual counseling, crisis intervention, and assistance to patients and their families with the preparation of advance directives, funeral planning issues, and the transfer of responsibilities related to legal and fiscal matters.
• Coordinate and facilitate access to community resources based on the specific needs of the patient and their family.

Job Qualifications:
Education: You must hold a Bachelor's or Master's Degree in Social Work from a State accredited program.
Licensure: You must have State Certification in Social Work.
Experience: We require at least one (1) year of experience as a social worker in a healthcare agency.
