LPN Home Health Nurse

Responsibilities:

• Deliver direct patient care based on the established Plan of Care
• Propose modifications to the Plan of Care to the Supervising RN in response to patient care concerns that are identified
• Create accurate and prompt documentation that aligns with the Plan of Care
• Effectively communicate with team members, patients, and families to achieve optimal patient outcomes

Requirements:

• Hold a valid license as a Licensed Practical Nurse (LPN)
• COVID vaccination is mandatory
• Graduated from an accredited nursing school
• Possess at least 1 year of clinical experience, preferably in home care or acute care settings
• Have a valid Driver's License, auto insurance, and reliable means of transportation
• Demonstrate excellent observation, problem-solving, and communication skills
• Exhibit compassion and integrity
