LPN Home Health Nurse

Responsibilities:

• Deliver direct patient care based on the predetermined Plan of Care
• Propose modifications to the Plan of Care to the overseeing Registered Nurse when addressing identified patient care concerns
• Produce accurate and timely documentation in alignment with the Plan of Care
• Effectively communicate with all members of the team, patients, and their families to achieve optimal patient outcomes

Requirements:

• Active license as a Licensed Practical Nurse (LPN)
• Satisfactory completion of an accredited nursing school
• At least 1 year of clinical experience; preference for experience in home care or acute care settings
• Valid Driver’s License, auto insurance, and dependable transportation
• Strong observation, problem-solving, and communication skills
• Demonstrated compassion and integrity
