Licensed Practical Nurse Hospice

Location: Newton, MA

LPN Hospice Nurse

Job Duties:

  • Administer direct patient care in accordance with the established Care Plan
  • Discuss potential modifications to the Care Plan with the overseeing Registered Nurse based on identified concerns regarding patient care
  • Maintain accurate and prompt documentation that aligns with the Care Plan
  • Facilitate effective communication with members of the Hospice team, patients, and families to facilitate optimal patient outcomes

Qualifications:

  • Active license as a Licensed Practical Nurse (LPN)
  • Graduate of an accredited nursing school
  • At least 1 year of prior experience in Hospice care
  • Valid Driver's License, reliable transportation, and auto insurance
  • Exceptional skills in observation, problem-solving, and communication
  • Exhibit compassion and maintain integrity in all interactions
