Expectations and Duties:

• Regular monitoring of patients' vital signs.
• Ensuring patients' adherence to medication schedules.
• Observing and documenting patients' progress during treatment.
• Providing progress updates to patients' families.

Conduct follow-up visits with patients requiring home health services.

Qualifications:

• Minimum one year of active clinical experience.
• Active LVN license in the state of Oregon.
• Valid driver's license with a clean DMV record.
