  • The LPN should possess the knowledge and skills required to provide appropriate care for patients of different age groups. They must demonstrate an understanding of growth and development principles across the lifespan, assess patient data effectively, interpret information to identify patient needs based on age-specific requirements, and deliver care to adult and geriatric patients.
  • This role involves providing both direct and indirect skilled nursing care to assigned residents. The LPN will adhere to established policies and procedures under the supervision of a Registered Nurse. They will maintain collaborative relationships with other nursing staff, physicians, and ancillary staff, while also continuously enhancing their knowledge and clinical expertise through ongoing education and in-service programs. The LPN will be responsible for providing care to geriatric residents.

    Job Duties:
    - Continuously collect relevant data related to the physical and psychosocial condition of residents, and communicate findings to the RN and other members of the Interdisciplinary Team.
    - Contribute to the development of an age-appropriate and culturally relevant resident care plan, involving the resident/family and healthcare team in the process, and reflecting nursing diagnoses and interventions to achieve desired outcomes while promoting a healthy and safe environment.
    - Recommend revisions to the care plan based on evaluation of resident outcomes, and coordinate referrals to ensure comprehensive care. Collaborate with the resident/family and Interdisciplinary Team within the scope of nursing practice, prioritizing customer satisfaction.
    - Maintain up-to-date knowledge and competency in Nursing Practice. Participate in the mentoring and orientation of staff. Consider safety, cost, and impact factors when planning and delivering care. Under the guidance of the RN, provide leadership in the professional practice setting, demonstrating knowledge and competency.
    - Occupational exposure to bloodborne pathogens is expected. Required tasks may involve contact with blood, body fluids, or tissues.
