LPN Weekend Intake Coordinator

Location: Jericho, NY

LPN Home Health Weekend Intake Coordinator

Roles and Responsibilities:

  • Weekend availability required with a minimum of 8 hours per day on Saturdays and Sundays
  • Managing and processing all referrals for Medicare Certified Home Health services
  • Evaluating and assessing clinical appropriateness and financial reimbursement for intake requests
  • Ensuring accuracy of all intake data
  • Demonstrating a comprehensive understanding of Skilled Home Health services during communication with referral sources and team members
  • Providing exceptional customer service
  • Collaborating closely with scheduling, Billing, and Quality departments

Qualifications:

  • Hold a valid New York Licensed Practical Nurse (LPN) license
  • At least 1 year of experience in a Certified Home Health Agency (CHHA)
  • Proficient in computer skills and data entry
  • Excellent interpersonal and customer service skills
