  • Coordinate all Medicare assessments, care planning, and reimbursement activities for the facility to ensure compliance with regulatory agencies, under the guidance of the Director of Nursing-LTC.
  • Manage MDS completion in accordance with regulatory requirements, including interpreting new standards, scheduling and communicating schedule to IDC team, attending care plan meetings as needed, ensuring completion of all MDS assessments, completing assigned MDS assessments, ensuring completion of CAAs, and submitting MDS data per regulations. Monitor and interpret Quality Measures.
  • Efficiently coordinate reimbursement activities for the facility, including making reimbursement decisions, completing denial protocols, and maintaining records. Act as a liaison between Nursing, Therapy, and Resident Accounts.
  • Provide documentation oversight and ensure compliance with reimbursement guidelines and requirements, identify documentation needs and review with the Nurse Manager/DON, and provide appropriate in-service training. Collect MDS census data for Medicaid reimbursement.
  • Have familiarity with reimbursement documentation requirements. Possess knowledge of New York State Department of Health regulations and survey process, as well as MDS and care planning experience.

  • Graduate from a Registered Professional Nursing Program.



  • Hold a current New York State Registered Professional Nurse license.
  • Certification in MDS and PRI; BLS through RQI upon hire.


  • Demonstrate the knowledge and skills necessary to provide age-appropriate care for the patients served on the assigned unit. Understand the principles of growth and development throughout the lifespan. Assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements. Provide care for adult and geriatric patient groups.

Exposure Control


  • No reasonable anticipation of occupational exposure to bloodborne pathogens. Normal work routine does not involve exposure to blood, body fluids, or tissues.

Ideal Candidate

  • The ideal candidate will have experience as an MDS Coordinator in a non-profit LTC facility.
  • Familiarity with documentation requirements for reimbursement is preferred.
  • Possess knowledge of New York State Department of Health regulations and survey process.
  • Hold a current New York state Registered Professional Nurse License.
