PACE MEDICAL DIRECTOR
MEDICAL DIRECTOR - PACE (All-Inclusive Care for the Elderly) program seeks to empower seniors to maintain their independence and community engagement by delivering a comprehensive range of medical services. We are embarking on the launch of a PACE program in Fresno, CA. In this role, as the Medical Director, you will operate at the heart of an IDT, providing personalized care. PACE presents an exceptional opportunity for clinicians, as it enables significant engagement with patients and allows for genuine caregiving, free from the misaligned incentives associated with fee-for-service models.
Primary Job Responsibilities:
- Deliver primary care and evaluate and treat acute illnesses for a panel of approximately 150 participants
- Conduct comprehensive assessments for participants and collaborate with the IDT to develop individualized care plans for each participant
- Actively engage as a member of the IDT, ensuring effective communication regarding participant updates, contributing to care planning arrangements, and coordinating around-the-clock care delivery
- Promote collaborative care coordination with network providers and IDT members, particularly concerning hospital discharge, short-term care, and long-term placement
- Provide direction and support to Nurse Practitioners
- Oversee participants' utilization of medical specialists and inpatient care
- Participate in rotating night and weekend on-call duties
- Supervise all participant care in the nursing home, conducting regular visits in accordance with PACE standards and participants' needs
- Analyze outcome and utilization data, including adverse events, hospitalizations, and specialist use, in collaboration with the IDT to identify opportunities for enhancing care and operations
Minimum Job Requirements:
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
- Master's or doctorate degree
- Board Certification in Internal Medicine or Family Practice
- Preferred Board Certification in Geriatrics
- Active unrestricted California Physician License
- State CDS Certification and DEA Registration
- Valid state driver's license with a clean DMV record
- Recent vehicle insurance documentation required
- Outstanding clinical, organizational, and communication skills applicable in a setting serving frail, elderly participants, their families, and the IDT
- Up-to-date knowledge of chronic care/geriatric issues and best practices

Salary: $260,831.32 - $339,274.69 per year
