MEDICAL DIRECTOR / PRIMARY CARE
PRIMARY CARE DIRECTOR
We are seeking a Primary Care Director to lead our team in Stockton, CA. As a part of our commitment to providing exceptional care, we offer a unique program called PACE (All-Inclusive Care for the Elderly) that allows seniors to continue living in their homes and communities while receiving comprehensive medical services. Our team operates under a collaborative interdisciplinary approach, ensuring that our participants receive holistic care and support.
In this role, you will work closely with the Chief Medical Officer and be responsible for providing primary care to our participants. You will play a key role in the evaluation and development of individual care plans, while collaborating with the interdisciplinary team to ensure seamless care delivery. PACE offers an ideal environment for clinicians to connect with and provide patient-centered care, free from the limitations of fee-for-service models.
Responsibilities:
- Deliver primary care and assess acute illnesses for a panel of approximately 150 participants
- Conduct comprehensive assessments and collaborate with the interdisciplinary team to develop personalized care plans
- Actively engage with the interdisciplinary team, communicating participant updates and coordinating care arrangements
- Work closely with network providers and the team to ensure effective care coordination, especially during hospital discharge and placement transitions
- Provide guidance and support to Nurse Practitioners
- Oversee participants' utilization of medical specialists and inpatient care
- Participate in rotating night and weekend on-call schedule
- Regularly visit and supervise participants in nursing home settings based on PACE standards and participant needs
- Analyze outcome and utilization data, in collaboration with the team, to identify areas for improvement in care and operations
Requirements:
- Completion of an accredited residency program in Internal Medicine or Family Practice
- Master's or doctorate degree
- Board Certification in Internal Medicine or Family Practice
- Preferably Board Certification in Geriatrics
- Active California Physician License with no restrictions
- State CDS Certification and DEA Registration
- Valid state driver's license with a clean driving record
- Proof of recent vehicle insurance
- Excellent clinical, organizational, and communication skills in serving elderly participants and working with the interdisciplinary team
- Up-to-date knowledge of chronic care and geriatric issues, as well as best practices

Salary: $260,831.32 - $339,274.69 per year
