MEDICAL DIRECTOR / PRIMARY CARE

PACE MEDICAL DIRECTOR

Are you passionate about providing comprehensive medical care to seniors? Join our team as a Medical Director for the PACE (All-Inclusive Care for the Elderly) program in Stockton, CA. Our program emphasizes holistic treatment and we are dedicated to keeping seniors living safely in their communities. As the Medical Director, you will play a central role in coordinating care within an interdisciplinary team. This is an ideal opportunity for a clinician who wants to put patient care first, without the constraints of a fee-for-service model.

Key Responsibilities:

  • Deliver primary care and evaluate acute illnesses for a panel of approximately 150 participants
  • Conduct comprehensive assessments and collaborate with the interdisciplinary team to create personalized care plans for each participant
  • Collaborate with the team to communicate participant updates, ensure effective care coordination, and facilitate 24-hour care delivery
  • Work with network providers and team members to coordinate hospital discharge, short-term, and long-term care
  • Provide guidance to Nurse Practitioners
  • Supervise participants' utilization of medical specialists and inpatient care
  • Participate in rotating night and weekend call schedule
  • Oversee care for participants in nursing homes, conducting regular visits as needed
  • Analyze outcome and utilization data to identify opportunities for improved care and operations, in collaboration with the interdisciplinary team

Requirements:

  • Completed residency program accredited by the Accreditation Council for Graduate Medical Education
  • Master’s degree or doctorate degree
  • Board Certification in Internal Medicine or Family Practice
  • Preferred Board Certification in Geriatrics
  • Active California Physician License without restrictions
  • State CDS Certification and DEA Registration
  • Valid state driver’s license with a clean DMV record
  • Recent vehicle insurance documentation
  • Excellent clinical, organizational, and communication skills, especially when working with frail, elderly participants and their families
  • Up-to-date knowledge of chronic care and geriatric best practices

Salary: $260,831.32 - $339,274.69 per year
