PRIMARY CARE PROVIDER / NURSE PRACTITIONER
Modesto, CA

Under the guidance of the Medical Director, the Nurse Practitioner collaborates with Physicians and the IDT to deliver assessments, planning, primary care, and continuous medical education to participants.

Key Responsibilities:

• Conduct comprehensive in-person assessments, including initial, semi-annual, and unscheduled assessments, and provide ongoing medical coverage for participants
• Work with physicians and IDT to create individualized care plans for each participant
• Cooperate with physicians and IDT in developing discharge strategies for acute or long-term care settings
• Ensure regular visits and maintain the care of participants in nursing homes based on their specific needs
• Collaborate with physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination
• Review outcomes and utilization data, such as hospitalizations and specialist usage, under the guidance of the Medical Director, in order to establish best practices and policy enhancements
• Participate in rotating night and weekend on-call duties

Requirements:

• Completion of an Accredited School of Nursing
• Certificate as a Nurse Practitioner with Prescriptive Authority
• Current RN State License
• Minimum of two years of experience working with the senior population
• Up-to-date knowledge of chronic care/geriatric issues and best practices
• Valid State Driver’s License with a clean DMV record
• Copy of recent Vehicle Insurance must be provided

SKILLS AND CERTIFICATIONS

• Nurse Practitioner certification with Prescriptive Authority
• Current California license
• Preferred experience in geriatrics
• Minimum of 2 years of Nurse Practitioner experience
