Salary: $145,831.32 - $169,274.69 per year

The Nurse Practitioner plays an integral role in the medical team, working closely with Physicians and the IDT to provide assessments, create care plans, and deliver primary care to patients. This position is located in the vibrant city of Stockton, California.

Key Responsibilities:

  • Conduct comprehensive in-person assessments, including initial, periodic, and unscheduled evaluations, to address the medical needs of patients
  • Collaborate with Physicians and IDT to develop personalized care plans for each individual
  • Coordinate discharge planning from acute or long-term care settings
  • Ensure the well-being of nursing home residents through regular visits based on their specific requirements
  • Work closely with Physicians, community-based PACE physicians, and IDT to ensure efficient care coordination
  • Review outcome and utilization data, such as hospitalizations and specialist usage, to identify opportunities for policy improvements
  • Participate in rotating night and weekend on-call duties


  • Graduation from an Accredited School of Nursing
  • Nurse Practitioner certificate with Prescriptive Authority
  • Active RN State License
  • Minimum of two years of experience working with seniors
  • Up-to-date knowledge of chronic care and geriatric best practices
  • Valid State Driver's License with a clean DMV record
  • Proof of recent Vehicle Insurance coverage

Skills and Certifications:

  • Nurse Practitioner certification with Prescriptive Authority
  • California state licensure
  • Experience in geriatrics (preferred)
  • Minimum of 2 years of Nurse Practitioner experience
