NURSE PRACTITIONER / PRIMARY CARE

PRIMARY CARE NURSE PRACTITIONER - YOUR GATEWAY TO WELLNESS

Under the guidance of the Medical Director, the Nurse Practitioner works harmoniously with Physicians and the Interdisciplinary Team (IDT) to deliver comprehensive assessments, personalized care plans, and continuous medical education to participants.

Key Responsibilities:

  • Conduct thorough in-person assessments, including initial, semi-annual, and unscheduled visits, and offer continuous medical support to participants
  • Collaborate with physicians and IDT in devising tailored care plans for each participant
  • Work with physicians and IDT to ensure smooth discharge planning from acute or long-term care settings
  • Assist in the ongoing management of participants residing in nursing homes, making regular visits as per their specific needs
  • Foster effective care coordination by working closely with physicians, community-based PACE physicians, and IDT
  • Analyze outcomes and utilization data, such as hospitalizations and specialist usage, under the guidance of the Medical Director to establish best practices and policy enhancements
  • Participate in night and weekend on-call rotation

Qualifications:

  • Graduated from an Accredited School of Nursing
  • Certification as a Nurse Practitioner with Prescriptive Authority
  • RN State License
  • Minimum of two years of experience serving the senior population
  • Up-to-date knowledge of chronic care/geriatric issues and industry best practices
  • Valid State Driver's License and a clean DMV record
  • Must provide a copy of recent Vehicle Insurance

Skills and Certifications:

  • Certification as a Nurse Practitioner with Prescriptive Authority
  • California license
  • Preferred experience in geriatrics
  • 2 years of Nurse Practitioner experience
