PRIMARY CARE NURSE PRACTITIONER
San Jose, CA

Annual Salary: $145,831.32 - $179,274.69

Under the guidance of the Medical Director, the Primary Care Nurse Practitioner collaborates with Physicians and the Interdisciplinary Team (IDT) to deliver comprehensive assessments, care planning, primary healthcare, and ongoing medical education to participants.

Key Responsibilities:

  • Conduct comprehensive in-person assessments, including initial, semi-annual, and unscheduled assessments, and ensure continuous medical coverage for participants
  • Work with physicians and IDT to develop personalized care plans for each participant
  • Collaborate with physicians and IDT in the discharge planning process from acute or long-term care settings
  • Provide regular nursing home visits to maintain participant care based on individual needs
  • Coordinate and oversee effective care management in collaboration with physicians, community-based PACE physicians, and IDT
  • Analyze outcomes and utilization data, such as hospitalizations and specialist usage, directed by the Medical Director and implement best practices and policy improvements
  • Participate in rotational night and weekend call schedule

Job Requirements:

  • Graduation from an Accredited School of Nursing
  • Certification as a Nurse Practitioner with Prescriptive Authority
  • Valid RN State License
  • Minimum of two years of experience working with older adults
  • Current knowledge of chronic care and geriatric issues, along with best practices
  • Active State Driver’s License with a clean DMV record
  • Recent Vehicle Insurance copy is required

SKILLS AND CERTIFICATIONS

  • Certified as a Nurse Practitioner with Prescriptive Authority
  • Current California State License
  • Experience in Geriatrics (preferred)
  • Minimum of 2 years of Nurse Practitioner experience
