PRIMARY CARE PROVIDER / NURSE PRACTITIONER
Stockton, CA

Compensation: $145,831.32 - $169,274.69 per year

Under the guidance of the Medical Director, the Nurse Practitioner collaborates with Physicians and the Interdisciplinary Team (IDT) to deliver assessments, create care plans, provide primary care, and deliver continuing medical education to participants.

Key Responsibilities:

• Conduct comprehensive in-person assessments, including initial, semi-annual, and unscheduled assessments, and provide continuous medical coverage to participants
• Collaborate with physicians and the IDT to develop individualized care plans for each participant
• Assist in discharge planning from acute or long-term care settings in coordination with physicians and the IDT
• Ensure ongoing care of participants in nursing homes by making regular visits based on participant requirements
• Work in collaboration with physicians, community-based PACE physicians, and the IDT to oversee and ensure efficient care coordination
• Review outcomes and utilization data, including hospitalizations and specialist use, as directed by the Medical Director, to establish best practices and policy enhancements
• Participate in rotating night and weekend call schedules

Requirements:

• Graduation from an Accredited School of Nursing
• Certificate as a Nurse Practitioner with Prescriptive Authority
• Valid RN State License
• Minimum of two years of experience working with the elderly population
• Up-to-date knowledge of chronic care/geriatric issues and best practices
• Valid State Driver’s License with a clean DMV record
• Copy of current Vehicle Insurance required

Skills and Certifications:

• Certification as a Nurse Practitioner with Prescriptive Authority
• Active California nursing license
• Experience in geriatrics (preferred)
• At least 2 years of Nurse Practitioner experience
