  • Conduct evaluations and create individualized treatment plans based on residents' medical diagnoses.
  • Implement treatment strategies to enhance functional independence and help residents achieve their goals.
  • Assess and document residents' progress in detailed re-certifications, progress reports, and daily notes.
  • Safeguard confidential information while maintaining resident confidence.
  • Ensure accurate documentation of resident care services by reviewing and auditing records.
  • Supervise occupational therapy assistants to provide optimal patient care.
  • Participate in care plan meetings and contribute to documentation.
  • Collaborate with the Rehabilitation Director to develop and organize OT-related reports and data tracking.
  • Assist with departmental projects as required.
  • Graduate of an accredited Occupational Therapy Program.
  • Valid license to practice Occupational Therapy in the state of New York.
  • Prior experience is desirable.
