Responsibilities:
- Conduct comprehensive evaluations and formulate treatment plans based on medical diagnoses of residents
- Implement treatment plans to enhance residents' functional independence and achieve their goals
- Thoroughly assess and document residents' progress through recertification, progress reports, and daily notes
- Ensure confidentiality and security of residents' information
- Maintain accurate records of resident care services by reviewing records regularly
- Provide clinical supervision to occupational therapy assistants
- Participate in care plan meetings and document relevant information
- Produce OT reports and track data as directed by the Rehab Director
- Assist in departmental projects as required
Requirements:
- Graduation from an accredited Occupational Therapy Program
- Valid license to practice Occupational Therapy in the State of New York
- Prior experience is advantageous
