Occupational Therapist, OT(Home Health) Full-time/Per-Diem
Position: Home Health Occupational Therapist
Status: Full-time/Per-Diem
We are seeking a highly skilled and dedicated Occupational Therapist to provide exceptional Occupational Therapy services.

ESSENTIAL RESPONSIBILITIES
- Conduct comprehensive assessments, diagnostic tests, and effective treatments for clients under medically approved Plans of Care.
- a. Complete assessments promptly and in compliance with organizational policies.
- b. Design individualized treatment programs and establish goals for enhanced functionality.
- c. Share assessment findings and goals to contribute to the development of client care plans and obtain necessary physician orders.
- d. Thoroughly document all findings, plans, interventions, and client progress on designated forms, adhering to organizational guidelines.
- e. Collaborate with physicians, nursing Case Managers, and other staff through care plans, progress notes, and participation in care conferences to communicate plans and updates.
- Provide guidance and instruction to clients in therapeutic self-care and engaging in activities that improve independence and physical and cognitive functioning.
- Advise, consult, and educate clients, family members, and staff on appropriate treatment procedures and supportive activities.
- Design and/or order adaptive/assistive devices and splints to optimize functionality.
- Instruct clients and their families in customized programs designed specifically for each client.
- Adhere to organizational policies, procedures, and professional and community standards.
- a. Deliver services in accordance with relevant state, federal, and JCAHO standards.
- b. Attend meetings and required educational programs.
- c. Take responsibility for personal and professional development, and communicate learning/developmental needs to supervisor.
- Attend care conferences, make interdisciplinary referrals, and participate in care coordination as necessary.
- Participate in the Clinical Record Review Committee.
- Perform other duties and responsibilities as assigned.
- Conduct comprehensive assessments, diagnostic tests, and effective treatments for clients under medically approved Plans of Care.
Qualifications:
- Completion of an accredited occupational therapy curriculum jointly accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligibility for the National Registration Examination of the American Occupational Therapy Association.
- Current registration or license as required by state(s) of practice.
- Minimum of two (2) years of experience in a healthcare facility or equivalent.
- CPR certification.
- Excellent verbal and written communication skills, as well as strong interpersonal skills.
- Valid driver's license with auto insurance in accordance with state and organizational requirements. Demonstrated safe driving record and reliable transportation.
