Occupational Therapist, OT(Home Health) Full-time

Key Responsibilities:
- Evaluate clients and conduct diagnostic tests, skilled treatments, and ongoing evaluations in accordance with approved care plans.
- a. Conduct assessments efficiently and according to our policies.
- b. Develop treatment programs and establish goals for improved function.
- c. Share assessment findings and goals to support the development of care plans and obtain necessary physician orders.
- d. Document all findings, interventions, and client progress accurately and submit timely reports.
- e. Communicate plans and changes with physicians, nursing Case Managers, and other staff members involved in care.
- Provide clear guidance and instructions to clients on therapeutic self-care and activities aimed at enhancing independence and physical and cognitive functioning.
- Advise, consult, and educate clients, their families, and other staff members on appropriate treatment procedures and supportive activities.
- Design and/or order appropriate adaptive/assistive devices and/or splints to support optimal functioning.
- Instruct clients and families on specific program requirements.
- Adhere to Agency policies, professional standards, and community guidelines.
- a. Deliver services in compliance with state, federal, and JCAHO standards.
- b. Attend meetings and required educational programs.
- c. Take responsibility for personal and professional development and communicate learning needs to supervisor.
- Participate in care conferences, initiate interdisciplinary referrals, and engage in care coordination as necessary.
- Contribute to the Clinical Record Review Committee.
- Perform other related duties and responsibilities as required.
- Evaluate clients and conduct diagnostic tests, skilled treatments, and ongoing evaluations in accordance with approved care plans.
Qualifications:
- Graduate of an accredited occupational therapy program jointly accredited by the American Medical Association and the American Occupational Therapy Association, or eligible for the National Registration Examination of the American Occupational Therapy Association.
- Current registration or license as required by the state(s) of practice.
- Minimum of two (2) years of experience in a healthcare facility or equivalent setting.
- CPR certification.
- Excellent verbal and written communication skills, along with strong interpersonal abilities.
- Valid driver's license and automobile insurance that meets state and Agency requirements. Must have a safe driving record and reliable transportation.
