Occupational Therapist, OT(Home Health) Per-Diem

We are seeking a skilled Occupational Therapist who is dedicated to providing exceptional Occupational Therapy services.

PRIMARY RESPONSIBILITIES
- Conduct occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation for clients under a medically approved Plan of Care.
- a. Conduct timely assessments in compliance with Agency policies.
- b. Develop treatment programs and set goals for enhanced functionality.
- c. Share assessment findings and goals to contribute to the client care plan, and obtain necessary physician orders.
- d. Document all findings, plans, interventions, and client progress (outcomes) on designated forms in adherence to Agency guidelines.
- e. Communicate plans and changes with the physician, nursing Case Manager, and other Agency staff through the care plan, written progress notes, and participation in care conferences.
- Guide and instruct clients in therapeutic self-care and creative activities to improve independence and physical and cognitive functioning.
- Advise, consult, and educate clients, family members, and Agency staff on appropriate treatment procedures and supportive activities.
- Design and/or prescribe adaptive/assistive devices and/or splints to promote optimal functioning.
- Instruct clients and their families in client-specific programs.
- Adhere to Agency policies and procedures, as well as professional and community standards.
- a. Provide services in accordance with applicable state, federal, and JCAHO standards.
- b. Attend meetings and mandatory educational programs.
- c. Take responsibility for personal and professional development and identify learning/development needs to supervisor.
- Participate in care conferences, initiate interdisciplinary referrals, and collaborate in care coordination as necessary.
- Serve on the Clinical Record Review Committee.
- Perform other relevant duties and responsibilities as deemed necessary.
- Conduct occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation for clients under a medically approved Plan of Care.
Qualifications:
- Completion of an accredited occupational therapy program recognized jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligibility for the National Registration Examination of the American Occupational Therapy Association.
- Valid registration or license as required by the state(s) of practice.
- A minimum of two (2) years of experience in a healthcare facility or equivalent setting.
- CPR certification.
- Excellent verbal and written communication skills and strong interpersonal abilities.
- Licensed driver with automobile insurance that complies with state/Agency requirements. Must have a safe driving record and reliable transportation.
