Occupational Therapist, OT(Home Health) Per-Diem/Full-Time

Per-Diem / Full-Time Opportunity

We are seeking a skilled Occupational Therapist to deliver exceptional Occupational Therapy services.


    1. Conduct occupational therapy assessments, diagnostic tests, effective treatments, and continuous evaluation of patients under a medically approved Care Plan.
      • a. Ensure timely completion of assessments in compliance with Agency policies.
      • b. Develop treatment programs and establish goals for enhanced functionality.
      • c. Provide assessment findings and goals to contribute to the development of patient care plans and obtain specific physician orders, when necessary.
      • d. Thoroughly document findings, plans, interventions, and patient progress (outcomes) on designated forms, following Agency guidelines.
      • e. Communicate plans and changes to physicians, nursing Case Managers, and other Agency staff through care plans, written progress notes, and participation in care conferences.
    2. Guide and instruct patients in therapeutic self-care and engaging activities aimed at improving independence, physical well-being, and cognitive functioning.
    3. Offer advice, consultation, and training on appropriate treatment procedures and supportive activities to patients, family members, and other Agency staff.
    4. Design and/or order adaptive/assistive devices and/or splints to optimize level of functioning.
    5. Instruct patients and their families on specific programs tailored to their needs.
    6. Adhere to Agency policies, procedures, and professional and community standards.
      • a. Provide services in accordance with applicable state, federal, and JCAHO standards.
      • b. Attend meetings and required educational programs.
      • c. Take responsibility for personal and professional development and identify learning/developmental needs to supervisor.
    7. Participate in care conferences, initiate interdisciplinary referrals, and contribute to care coordination as needed.
    8. Serve on the Clinical Record Review Committee.
    9. Perform other related duties and responsibilities as required.


  • Graduate of an accredited occupational therapy curriculum, accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligible for the National Registration Examination of the American Occupational Therapy Association.
  • Hold current registration or license as required by state(s) of practice.
  • Minimum of two (2) years of experience in a healthcare facility or equivalent setting.
  • Valid CPR certification.
  • Demonstrate excellent verbal and written communication skills, as well as strong interpersonal skills.
  • Licensed driver with automobile insurance in accordance with state/Agency requirements. Provide evidence of a safe driving record and reliable transportation.
