Occupational Therapist, OT(Home Health) Per-Diem/Full-Time

Per-Diem / Full-Time Opportunity

We are seeking an Occupational Therapist who is passionate about delivering exceptional Occupational Therapy services.


    1. Conducts assessments, diagnostic tests, skilled treatments, and continuous evaluation of clients who are receiving services under an approved medically sanctioned Care Plan.
      • a. Ensures timely completion of assessments following Agency policy.
      • b. Designs a treatment program and establishes goals to enhance functionality.
      • c. Shares assessment findings and goals with the care team to contribute to the development of the client care plan and obtains specific physician orders, if necessary.
      • d. Documents all findings, plans, interventions, and client progress (outcomes) on designated forms and submits them according to Agency guidelines.
      • e. Communicates plans and updates to the physician, nursing Case Manager, and other Agency staff via the care plan, written progress notes, and participation in care conferences.
    2. Provides guidance and instructions to clients on therapeutic self-care and engaging in creative activities that aim to improve independence, physical and cognitive functioning.
    3. Advises, consults, and educates clients, family members, and other Agency staff on appropriate treatment procedures and supporting activities.
    4. Designs and/or orders appropriate adaptive/assistive devices and/or splints to facilitate the highest level of functioning.
    5. Teaches clients and their families about the specific program tailored to the individual's needs.
    6. Carries out job responsibilities in compliance with Agency policies and procedures, as well as professional and community standards.
      • a. Provides services in accordance with applicable state, federal, and JCAHO standards.
      • b. Attends meetings and required educational programs.
      • c. Takes responsibility for personal and professional development, and communicates learning and developmental needs to the supervisor.
    7. Attends care conferences, initiates interdisciplinary referrals, and actively participates in care coordination when necessary.
    8. Participates on the Clinical Record Review Committee.
    9. Performs other related duties and responsibilities as necessary.


  • Graduate of an accredited occupational therapy program, accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligible for the National Registration Examination of the American Occupational Therapy Association.
  • Holds current registration or license as required by the state(s) of practice.
  • Has a minimum of two (2) years of experience in a healthcare facility or equivalent setting.
  • CPR certification.
  • Demonstrates excellent verbal and written communication skills, as well as good interpersonal skills.
  • Licensed driver with a vehicle insured in accordance with state/Agency requirements. Can provide evidence of a safe driving record and reliable transportation.
