Position: Home Health Occupational Therapist

We are currently seeking a dedicated Occupational Therapist to join our team and provide exceptional Occupational Therapy services. The ideal candidate will be passionate about their work and committed to delivering high-quality care.


  • Conduct comprehensive occupational therapy assessments, diagnostic tests, and skilled treatments for clients following approved Plan of Care.
  • a. Ensure timely completion of assessments in accordance with company policies.
  • b. Develop personalized treatment programs and establish goals to enhance clients' functionality.
  • c. Share assessment findings and goals with the care team to assist in developing individualized care plans and obtain necessary physician orders.
  • d. Maintain accurate documentation of all findings, plans, interventions, and client progress according to company guidelines.
  • e. Collaborate with physicians, nursing Case Managers, and other agency staff to communicate care plans and updates.
  • Provide guidance and instruction to clients on therapeutic self-care and creative activities aimed at enhancing independence and physical and cognitive functioning.
  • Offer advice, consultation, and training on appropriate treatment procedures and supportive activities to clients, family members, and agency staff.
  • Create and order suitable adaptive/assistive devices and splints to optimize clients' functional abilities.
  • Instruct clients and their families on individualized therapy programs.
  • Adhere to agency policies, procedures, and professional standards.
  • a. Deliver services in compliance with state, federal, and JCAHO standards.
  • b. Participate in meetings and educational programs as required.
  • c. Take responsibility for personal and professional growth, identifying developmental needs and seeking supervision when necessary.
  • Participate in care conferences, interdisciplinary referrals, and care coordination as needed.
  • Serve on the Clinical Record Review Committee.
  • Perform any other duties and responsibilities as assigned or deemed necessary.


  • Graduate of an accredited occupational therapy program approved jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA); or eligible to take the National Registration Examination of AOTA.
  • Current registration or license as required by the state(s) of practice.
  • Minimum of two (2) years of experience in a healthcare facility or equivalent setting.
  • CPR certification.
  • Strong verbal and written communication skills, along with good interpersonal skills.
  • Valid driver's license and reliable transportation with automobile insurance in accordance with state/agency requirements, along with a safe driving record.
