Job Description:
As a Patient Services Coordinator, your responsibilities will include:
  • Supporting effective communication and coordination of care needs reported by case managers
  • Ensuring continuity of care for patients, maintaining updated schedules, and delivering quality care through the clinical team
  • Facilitating case conferences and taking responsibility for follow-up actions related to meetings
  • Coaching and mentoring clinicians and CSS role, addressing development needs and KPIs
  • Acting as the main point of contact for personnel concerns and developing solutions for assigned team
  • Supervising and directing team members, assigning visits, and ensuring timely completion
  • Managing the site in the absence of higher-level management
  • Overseeing clinical, administrative, and recordkeeping functions
  • Responding to phone referrals professionally and compassionately
  • Participating in on-call nursing schedule and providing skilled bedside care in patients' homes

Requirements for Qualified Candidates:
  • Registered nurse with current licensure to practice nursing in Waynesboro, GA
  • Possess and maintain CPR certification
  • Previous experience in a home care setting with management or supervisory experience preferred
  • Proven ability to work effectively within an interdisciplinary team
  • Must have a valid driver's license, insured automobile, and meet organizational requirements
  • Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements
  • Excellent observation, nursing judgment, and communication skills
