Key Responsibilities

The Registered Nurse Patient Care Coordinator plays a vital role in overseeing and organizing clinical services. This includes coordinating and leading a multidisciplinary team to ensure exceptional care for patients receiving home health services. The Registered Nurse Patient Care Coordinator is responsible for ensuring compliance with physician-prescribed care plans, as well as all relevant state and federal laws and regulations.

License Requirements

  • Must hold a valid RN license in the state of practice
  • CPR certification must be current
  • Must possess a valid Driver's License, vehicle insurance, and have access to a reliable vehicle or public transportation
