Per-Diem Physical Therapist, PT

We are seeking a Physical Therapist who will evaluate and treat patients, based on physician orders, using therapeutic procedures aimed at restoring muscle strength, coordination, body alignment, pain relief, and improving movement and function. As a member of our interdisciplinary team, the Physical Therapist will also provide support and information within the realm of Physical Therapy.


1. Review the available information on each patient and assess how it impacts the treatment process.

2. Conduct a comprehensive functional assessment of the patient and document the findings using standardized procedures. This assessment should include an evaluation of their home/vocational needs and equipment requirements.

3. Develop an individualized treatment plan for each patient, focusing on their specific needs and achievable functional goals.

4. Adhere to evaluation, treatment, and documentation guidelines, ensuring that all department records are accurately and promptly maintained.

5. Train and supervise Assistants in the proper treatment procedures while closely monitoring their interactions with patients.

6. Record a progress note in the patient's medical record after each treatment session.

7. Modify treatment plans as necessary throughout the treatment process.

8. Create a clear and concise discharge summary documenting the course of treatment.

9. Familiarize patients with our program services, treatment procedures, and any potential risks involved.

10. Collaborate with nursing staff, patients, and families to maximize the patient's functional abilities.

11. Instruct patients, their families, and caregivers on exercise programs and therapeutic procedures to be continued at home after treatment.

12. Assist in the referral process if additional services are required after the patient is discharged, maintaining knowledge of community resources.

13. Attend and actively participate in rehabilitation team conferences to discuss patient progress, problems, and needs.

14. Attend other department/facility meetings, as required.

15. Seek out and attend continuing education opportunities relevant to your discipline and responsibilities to maintain clinical competency.

16. Contribute to the quality improvement process.

17. Promote the programs and services of Heritage Healthcare through interactions with the community.

18. Adhere to the regulatory and professional standards and guidelines set by Heritage Healthcare.

19. Provide feedback to the Rehab Manager on the clinical performance of Assistants, when requested and appropriate.

20. Adhere to facility policies, participate in facility quality improvement programs, and prioritize safety.

21. Perform any additional duties necessary to provide quality care to patients.

22. Understand the reimbursement processes of different payers and allocate therapy resources efficiently based on the patient's goals and discharge plans.

23. Demonstrate knowledge of OBRA regulations, HIPAA, Local Coverage Determination, and third-party reimbursement guidelines.

24. Support the company's commitment to Compassionate Care and maintain a professional image to inspire confidence in the Rehab Program.

25. Carry out duties independently and seek direct supervision when needed. Be willing to assist others.

26. Maintain the expected productivity level discussed for your position at your particular facility/facilities.

1. Hold a Bachelor/Masters of Science degree in Physical Therapy.

2. Possess a current license in the state of practice.

3. Prefer supervision, training, or experience in a setting that serves a similar age/type of population as our facility.

4. Certification, if applicable, and demonstrated competency in specific areas of specialization.

5. Display excellent oral and written communication and interpersonal skills.

6. Demonstrate competence in applying physical therapy principles, methods, materials, and equipment.

7. Ability to assess patient needs and develop and implement comprehensive care plans.

8. Familiarity with accreditation standards and compliance requirements.

9. Capability to perform the following activities safely for prolonged periods:

• Standing: Maintaining an upright position on the feet, especially for extended periods

• Pushing: Applying pressure to move objects forward or away from the source of force, with a push or pull force of up to 35 lbs.

• Lifting: Raising objects from lower to higher positions, or moving objects from one position to another

• Independently lift up to 50 lbs of force. May require assistance from another person or a mechanical lift to lift over 50 lbs.

• Stooping: Bending the body forward and downward from the waist, hips, and knees for an extended period of time.

• Reaching: Extending hands and arms in any direction away from the body.

• Balancing: Maintaining body equilibrium to prevent falling when walking, standing, or crouching on narrow, slippery, or moving surfaces.
